Health Innovation KSS is delivering the Blood Pressure Optimisation Programme, with the aim of preventing heart attacks, strokes and dementia in patients with hypertension, by improving the management in primary care. The programme aims to do the following:

  • Support primary care to implement the UCL Partners Proactive Care Framework for hypertension (and other CVD related conditions) to Risk stratify their patient to enable them the highest risk patients to be seen first.
  • Optimise patients with hypertension who are not treated to target.
  • Support detection of patients with undiagnosed hypertension through case finding.
  • Encourage home monitoring and innovation to enable patients to manage their blood pressure.
  • Reduce health inequalities by targeting those who are likely to be at greatest risk of CVD.

High blood pressure is the leading modifiable risk factor for heart and circulatory disease in the UK and it accounts for approximately half of all strokes and ischemic heart disease events globally. It is estimated that 28% of adults in the UK have high blood pressure. However, it is estimated that at least half of those who are diagnosed are not receiving effective treatment, and up to 4.8 million remain undiagnosed.

There are also significant inequalities associated with high blood pressure. The prevalence is 30% more likely in the most deprived areas of England compared to the least deprived.
